Flexible Intramedullary Nailing of the Fracture of Radial Shaft in Adults

Abstract
Purpose: To assess the results of the flexible intramedullary nailing for the radial shaft fracture in adults and to evaluate the technical factors thereof.

Materials and Methods: Materials for this study are 20 adults who had a single radial shaft fracture or internal fixation (plate and screws) of ulnar fractures in both forearm bones fracture with more than 1 year follow-up out of 82 adults of forearm fractures discovered during periods from August 1996 to December 2005. After classifying into four groups regardless of fixatives as follows ; radial shaft fractures with 1 flexible intramedullary nail (Group I), the cases of 2 flexible intramedullary nail (Group II), and as for the subgroups of the preceding cases, the group of three-point fixation with more than 1 fixative in subchondral bone of radial head or cortex of radial neck (subgroup-a) and that of short fixative whose head is out of reach to radial neck (subgroup-b), we deduced the technical factors effecting the results with the evaluation the outcome of the treatment for each cases through Anderson¡¯s method.

Results: Satisfactory outcomes were attained for 3 cases out of 4 (75%) in group Ia, 1 out of 3 (33%) in group Ib, 7 out of 7 (100%) in group IIa, 4 out of 6 (67%) in group IIb. Nonunion was found in 1 case of group Ib and revision surgery was performed with plate and screws fixation. Angular deformity was also found in the same group.

Conclusion: Insertion of two fixatives is recommendable to gain a stability of fracture site in the flexible intramedullary nailing of the radial shaft fractures in adults. When unavailable, use of a fixative of enough length to get three-point fixation is helpful in getting satisfactory results.
